Your Visit

Before your visit, please review the following

For your appointment:

  • Arrive 15 minutes prior to you appointment to allow time for Parking and Paperwork.
  • Bring your Insurance Card. A copy will be needed.
  • Call 510.848.4800 EXT 317 if you need to change/cancel your appointment or email us at audiology@ceid.org to avoid any fees.

Translation:
If you need an interpreter, we provide Spanish speaking interpreters. If you need an interpreter for a different language, please let us know.

Late Policy:
Due to our busy schedule, we will have to reschedule your appointment if you arrive more than 15 minutes late. Please do your best to be on time, or call us to reschedule!

Fees/Payment:
If payment is required during your visit, we accept credit card or cash only. Payment will be due on the day of service.

  • Por favor llegue 15 minutos antes de su cita para encontrar estacionamiento y llenar las formas necesarias.
  • Traiga su tarjeta de aseguranza. Necesitaremos una copia.
  • Llame al 510.848.4800 Ext. 317 o escribanos a audiology@ceid.org si necesita cambiar o cancelar su cita.
  • Si no podrá asistir a su cita, por favor llame o mande un correo electrónico 48 horas antes para evitar un cargo de $25 por cancelación.

  • Contra Costa Health Plan (CCHP) * prior authorization required
  • Alameda Alliance
  • California Children’s Services (CCS) * prior authorization required
  • Private Commercial Insurance
  • We also accept patients who would like to pay privately if insurance does not have out of network benefits.
